Rural communities are in need of people who are willing to do medical jobs in specialized fields. In the Midwest you have a situation where doctors move here specializing in things like podiatry or urology, only to realize that there may not be enough patients available to keep up with their finances and preferred style of living.

This is why you see some podiatrists in rural communities also attempting to get their certifications to be surgeons as opposed to merely just diagnosing foot issues they want an opportunity to operate on their patients, not only to help alleviate the pain and struggles that foot issues can case, but also to supplement their own income. Some podiatrists and other medical professionals do not want to risk becoming a surgeon because of the increased costs of medical malpractice insurance. Some podiatrists would just prefer the safety of diagnosing a fungus as opposed to cutting people open and dealing with all the pressure involved in preforming an operation.

They have seminars and courses that licensed physicians can attend in order to learn how to perform surgery and to reach the proper certification goals. The physicians can either pay for the courses/seminars at their own expense or a local medical association that they belong to may see a real need for a surgeon in the area and help cover some of the costs of the certification process. Sometimes in these rural communities out of the office issues can cost a physician their job and also hurt the community in the process. For example if a town has finally landed a trained and experienced urologist like they have wanted for years and yet he or she fired for a driving under the influence infraction then this can really be a disappointment. The urologist may have already developed a good working relationship with their patients and to have something like a DUI derail such progress is a real shame. You obviously don't want doctors showing up to work intoxicated, but the hospital should consider looking the other way in one time incidents where it doesn't appear to have effected job performance. This is particularly true when the need for medical professionals is so great.

You also don't have a lot of plastic surgeons or people who can perform botox injections. Maybe this stops people in smaller, rural communities from thinking that they are glamorous enough to make it to the big time. Some people may think you need to look a certain way in order to be a movie star and they want to get these operations done. It isn't a good idea to plant these kind of ideas into the minds of young people, but it would be fair for these kind of medical jobs to be there closer to home in order for them to have their options open. Hopefully young people will believe that they can break out of their small hometowns if they want to based on their own talent and charm as opposed to feeling the need to have a medical procedure.
