When a cosmetic surgeon performs a lower blepharoplasty, patients are offered a gentle intravenous sedation from their anesthetist. This surgery is capable, however, of being comfortably performed with only the use of a local anesthetic. For any patient who has fat bags under their lower eyelids that are prominent and only have a small amount of excess skin or muscle, the blepharoplasty can be performed with no external sutures. Instead, the plastic surgeon can place an incision on the inner side of the lower eyelid and any extra fat can be carefully removed. Eyelid tightening, or lateral canthoplasty is a procedure that is often done at the same time as lower eyelid surgery in order to tighten an extremely loose lower eyelid. With this surgery, you will be aided in restoring a more youthful position to your lower eyelid in relation to the iris. Other additional procedures that can be performed include fat grafting or fat transfer to the lower eyelid region, as well as lower eyelid laser resurfacing or chemical peel. Both of these may enhance the results of a lower eyelid lift or lower blepharoplasty. Laser eyelid surgery is a technique by which the CO2 laser replaces the scalpel for meticulous incision making.
