Since cohesive implants cannot be squeezed through a small incision (remember they are not liquid; they are a solid form), the incision required for cohesive gel implants is generally larger than saline and even silicone implants require. Larger incision size is definitely one of the disadvantages of cohesive gel implants.

Since cohesive gel implants only come in anatomical shapes (they are more full near the bottom than at the top), even a minimal amount of rotation can visibly affect the appearance of the breast. Surgery is the only way to correct a rotated cohesive breast implant, and it is recommended that a round implant be used to replace it.
