MAN BREAKS COLLAR BONE - LEADS TO THORACIC OUTLET SYNDROME

This was an interesting case, regarding a patient we will call Matt, for two reasons. First, Matt's secondary condition (TOS) was not properly diagnosed or treated. Second, we were able to help him quite successfully once we figured out what was going on.

When we have a trauma/accident severe enough to break a bone, chances are spinal bones (spinal vertebra) can be jarred out of their normal position.

This is what happened to Matt. He broke his collar bone which caused him to go into the emergency room for X-rays, and walked out with a sling and prescriptions to reduce pain and inflammation. This is Matt's story.

The problem was that, after a few weeks, he began to develop a secondary condition (TOS) with radiating arm pain, numbness, tingling and diminished circulation (cold hand) that scarred him.

CHANCES ARE...

Chances are if you are reading this, you have been diagnosed with Thoracic Outlet Syndrome or TOS for short. First of all, lets define it. TOS is a combination of pain in the neck and shoulder, numbness and tingling of the fingers, and weak grip.

The thoracic outlet is the area between the rib cage and collar bone. Bordered on three sides by the Trapezious muscle (on top), the Scalene muscles (on the side), and the collar bone (on the bottom).

Typically, the neck pain is associated with numbness and tingling into the last three fingers and the forearm. Carrying something heavy may make the pain worse. Poor circulation and weakness into the hand can be found.

KEY TO SUCCESS

The key to success with this condition is to diagnosis, treat and resolve the condition before it leads to cortisone injections or surgery. Yes surgery is one recommendation for TOS if unresolved. Surgery can involve removing muscles, and/or part of the clavicle or first rib.

Events leading to TOS: recent trauma (whiplash) like in a car accident, fractured clavicle, overuse syndrome for swimmers or baseball pictures, volleyball, weight lifters, etc. At its most serious stage, can cause stroke due to diminished blood supply.

Other causes involve poor posture like sitting in front of the computer leading to "head forward position", previous injury to the neck such as a car accident at some time in the past.

My training as a chiropractor is to look to the joints above and below the injury and trace it back to the spine to see if there is an involvement.

In Matt's case there was spinal involvement. It turns out that when he fell and broke his collar bone, the sudden muscular force also caused his neck to go out of alignment, without him really being aware of it. This caused further insult to the scalene muscles (also known as the "entrappers").

Nerves travel from the spinal cord in the neck, threw the scalene muscles, behind the collar bone, across the shoulder, down into the arm, hand and fingers. This is where Matt was getting numbness and tingling and diminished circulation.

On examination, his scalene muscles (located on the side of the neck) were severely inflamed, tight and now compressing or entrapping the exiting nerves causing his disability.

Medical treatment includes anti-inflammatory and pain medications, physical therapy and/or surgery to remove part of the rib.

CHIROPRACTIC CARE WORKS

This is a typical example of how even a secondary condition such as TOS can often times be effectively treated with chiropractic care. Appropriate stretching is also recommended to help prevent return of TOS.

And who is the most qualified doctor to determine if chiropractic care would help...a chiropractor of course.

In fact, if proper treatment is delayed the patient could well be predisposed to early degenerative changes and disc wear & tear all leading to more serious conditions later in life.
