Quests are always giving out items. With these items you can basically keep your gear updated at any time. Just do good rewarding quests.

Whenever you kill a creature in or outside an instance, it will always drop items for you. If youre lucky, you will get a rare item.

Auction House.

An other great, but costly way is to purchase items from the Auction House. As Ive said before, this can be expensive but its an effective way to keep your gear up to date.
